Tranexamic acid indirectly reduces bradykinin production by decreasing plasmin. Angioedema can be histamine or bradykin-mediated. Hereditary angioedema, ACE-inhibitor induced, and other idiopathic non-histamanergic causes are due to excessive bradykinin. Kallekrein and C1-esterase are important enzymes for bradykinin regulation These are also byproducts of the clotting cascade. That’s the downstream effect TXA has by reducing plasmin. Theres more evidence in maintenace therapy for hereditary angioedema, but smaller scale trials and case reports may show benefit for acute etiologies. Standard anaphylaxis therapies are given initially because of the presentation and can serve as a diagnostic. Fresh frozen plasma and C1-esterase inhibitors are also considered, but take coordination and time to procure. Guidelines recommend C1 esterase inhibitors, tranexamic 1gm IV over 10 minutes may have a role as a bridge for other treatments.
