Muscle rigidity is a key characteristic of neuroleptic malignant syndrome (NMS). NMS deals with abrupt alterations to dopamine, which is heavily involved with thermoregulation. Patients will be delirius, febrile, and have autonomic instability. Key risk factors include change in antidoomanergic/Parkinson regimens, parenteral adminstration, and/or polypharmacy. We’ve got options in the drug bank. Benzodiazepines are the mainstay for symptomatic control. Dantrolene is the only muscle relaxer I believe in; it inhibits calcium release from the endoplasmic reticulum causing muscle relaxation. We’d also give dopamine agonist in severe cases alongside dantrolene, such as bromocriptine and amantadine. Cool em cause they’ll be hot like pharmfax.
