These organisms can get pretty scary given their ability to rapidly produce resistance mechanisms and biofilms. They’re known to be hospital-aquired organisms and find their way to the immunocompromised. Let’s get you set up with an IV/PO Antibiotics for Pseudomonas starter pack. Fluoroquinolones are the only oral antibiotics that cover PsA and available as IV as well. Having one class of antibiotics that works tells you how complicated management can be.
IV deposits for Pseudomonas are beta-lactams, carbapenems, and aminoglycosides. Pip-tazo, ceftazadime, and cefepime are effective against Pseudomonas. Generally, beta-lactams are the best tolerated antibiotics for most infections. Aminoglycosides aren’t used as monotherapy since their not recommended first line or even definitive therapy..
Carbapenems, excluding ertapenem, are the next class of antibiotics for resistant Pseudomonas to common beta-lactams. Meropenem is commonly used for ESBL and Pseudomonas coverage. Antimicrobial stewardship is important since options start to get really limited with carbapenem-resistant pseudomonas species.
