Intralipid emulsion is standard therapy for local-anesthetic systemic toxicities. Intravenous lipid emulsion has been extrapolated as an antidote for non-local anestethic lipophilic drug poisonings. This therapy has been used for a variety of ingestions within the realm of the kitchen sink, including AV nodal blocking and psychotropic drugs. Intravenous lipid emulsion resuscitation derives from the lipid sink phenomenon, which hypothesizes that intralipids create an expanded lipid phase, forming a concentration gradient that pulls toxic lipophilic drugs from tissue. In the setting of true overdoses, we’re limited on proven antidotes but intralipid emulsion therapy can be considered after standard ACLS resuscitative measures have been exhausted for overdoses from lipophilic drugs.
