Target pathogens for epididmyitis and orchitis are dependent on patient risk factors. Men have a gradual onset of scrotal pain with UTI symptoms . Inflammation of the testes already sounds problematic.
Epididymitis and orchitis are mostly caused by bacterial infections. Treatments take into account high risk factors and age. Sexually transmitted disease pathogens, specifically Chlamydia and Gonorrorhae, are more common in men aged 14 - 35. These patients would receive standard prophylactic treatments of ceftriaxone 500 mg and doxycycline 100 mg bid x 10 days.
Men who practice insertive anal intercourse have exposure to more more broad gram negative organisms introduced; these patients will also receive levofloxacin 500 mg daily for 10 days. Partners within the past 60 days should be referred for evaluation.
Men greater than 35 or have low risk factors will receive empiric levofloxacin. Target pathogens are common enteric organisms, including E. coli. Narrow therapies once diagnostics finalize. Get it taken care of before it turns into an abscess. Yikes.
