The example we’ll use include anticholinergics and their toxidrome. This results from acetylcholine accumulation since anticholinergics block the activity of acetylcholine. Anticholinergics cause dry mouth, blurry vision, confusion, delirium, difficulty urinating, constipation, and fast heart rate. The class effect of anticholinergics can be remembered with this mneuomonic: red as a beet, dry as a bone, blind as a bat, mad as a hatter, hot as a hare, full as a flask. Benadryl, Zyrtec, Loratadin, Allegra, and Unison are common OTC anticholinergics used for allergies and sleep aids. Its important to note that many prescribed antidepressants, muscle relaxers, and antipsychotics have anticholinergic properties as a secondary mechanism. Severe cases may lead to seizures. We have an antidote called physostigmine that is indicated more for minimizing copious amounts of sedatives for acute agitation and delirium, and doesn’t have any anticonvulsant properties.
